What to Bring

  • health insurance card
  • medical care certificate (if any)
  • patient registration card (if you are a new patient, please fill out a registration form)
  • medication notebook (or app)
  • medicines you regularly take

Deposit

you will be asked to pay a deposit if you fall into one of the following conditions.

  • have forgotten to bring your health insurance card
  • conditions caused by a third party
  • self-injuries
  • traffic accident victim
  • occupational injuries

At your payment, you will receive a receipt. Bring the receipt to a reception counter (#3) later on to get refund.

First Visit Revisit (within 6 months)

10,000 yen 5,000 yen
traffic accident victim 15,000 yen
●pay the bill or get refund within 14 days at a reception counter #3 on weekdays (9:00 a.m. - 4:00 p.m.).
●Bring the deposit receipt and your health insurance card.
